Tuesday, May 26, 2026- Three workers were killed and another seriously injured after a tragic accident aboard a floating storage and offloading vessel operated by Petroliam Nasional Berhad off the coast of Terengganu, Malaysia.
The incident happened during lifeboat maintenance operations at the FSO Sepat platform when a rope or hook attached to the lifeboat reportedly failed, causing the workers to plunge into the sea. The victims were rushed to hospital but three were pronounced dead on arrival, while the surviving worker remains under medical observation.
Petronas confirmed the accident in a statement and said investigations are now underway alongside Malaysian authorities to determine exactly what caused the deadly failure.
Reports from local media indicate the workers had boarded the lifeboat as part of a routine offshore maintenance procedure before the securing mechanism unexpectedly gave way. The accident has already raised urgent concerns about safety standards and equipment reliability on offshore oil and gas facilities operating in the region.
The tragedy is likely to intensify scrutiny on offshore industrial safety practices across Malaysia’s energy sector, particularly involving contractor operations and emergency equipment maintenance.
